How to Fill out the International Travel Request form

This article is for employees who are traveling internationally.

Where to find the form?

1. The International Travel Request form can be found at the following link address: https://formstack.apu.edu/workflows/iac_travel_request

Approval Process: 

1. Fill out the fields as they are listed on the form and submit it

2. Wait for the form to be approved or denied by Dean or VP

3. Wait for the form to be given final approval or denial by the International Activities Committee
